BHS Athletic Packet Forms Important: Athletic Packets & Sports Physicals Students athletes must have an annual sports physical and complete all the forms in the BHS Athletic Packet. The forms can be obtained from the BHS Athletic Director Office or BHS Administration Office, This completed packet must be submitted to the BHS Athletic Office and cleared before the first day of tryouts. Sports physicals may be obtained from your private physician. |
