		Please print off and complete this grant application for BHS athletic team equipment/supplies/uniforms and turn into the Athletic Director.  The Athletic Director reviews all requests and makes recommendations to the Berkeley Athletic Fund (BAF).  The BAF meets to consider all requests and with the Athletic Director's recommendations decides on approval and the amount of funding.  Coaches are notified as soon as possible after the decision

		Please note:  The Berkeley Athletic Fund can only fund equipment/supplies/uniforms that will remain with the BHS team.  BAF does not fund coaches stipends nor out of town tournaments/training camps.  All items funded will be owned by the school and are not student property.  We cannot fund items that players will keep, such as caps or t-shirts or swimsuits.  We ask that a coach or parent be responsible for collecting equipment and uniforms at the end of the season so they can be returned to the team for next season.    Teams can use their BAF designated accounts (money from the parking lot, designated donations, etc) for items not covered by the BAF.
